Please note: Without the patient forms completed and returned to Dove Physical Therapy, LLC, physical therapy treatment cannot begin.
Only the initial evaluation can be performed.

Self-Payment Form

If you do not have health insurance coverage, the Self-Payment Option Agreement may be more suitable and flexible for you.  Self-payment gives you the freedom of choice to receive health care or physical therapy services that are deemed a medical necessity to improve your overall quality of life.  The length and type of treatment deemed necessary are jointly determined by you and the licensed physical therapist.  If you decide to pay privately for the services you receive from DovePT, click on the Self-Payment Form button below to access the Self-Payment Option Agreement Form that must be completed prior to the initial evaluation appointment.
